运动再学习结合活血化瘀治疗脑卒中偏瘫的临床研究

Effect of Motor Relearning Program Combining With Activateing Blood Circulation and Removing Blood Stasis in Patient with Hemiplegia After Stroke

目的 :探讨以运动再学习疗法为主的早期康复训练结合水蛭、血栓通活血化瘀治疗对脑卒中偏瘫患者神经功能缺损、运动功能及日常生活活动能力的影响。方法 :6 7例脑卒中偏瘫患者随机分成治疗组 (34例 )和对照组(33例 )进行了临床对照性研究 ,两组均常规进行神经内科的药物治疗 ,对照组采用运动再学习法的早期康复治疗方法 ;治疗组在使用运动再学习疗法的同时 ,加用水蛭干粉及血栓通活血化瘀治疗 ,并对每例患者在进入课题前和研究结束后的 3天内进行神经功能缺损、运动功能和日常生活活动能力的评定。结果 :两组前后比较患者临床神经功能缺损有显著性差异 (P <0 .0 1) ,运动功能和日常生活活动能力均明显改善 (P <0 .0 5 ) ,但治疗组临床神经功能缺损改善程度明显优于对照组 (P <0 .0 1) ,运动功能和日常生活活动能力改善程度明显优于对照组 (P <0 .0 5 )。结论 :运动再学习疗法结合水蛭、血栓通活血化瘀综合治疗方法对脑卒中偏瘫患者的运动功能恢复具有促进的作用 ,可明显降低患者的功能障碍 ,提高其生活质量。

Objective :To study the effect of motor relearning program combining with powder of leech and injection of PCM xueshuantong(血栓通)therapy on neurological deficits and motor functionin and ADL in stroke patients with early rehabilition.Methoods:67 patients were randomly divided into two groups: rehabilition group(34cases) and control group(33cases).Patients in the rehabilition group were rehabilitated with motor relearning program combining with activate blood circulation and remove blood stasis with powder of leech and injection of PCM xueshuantong therapy,while those in control group were rehabilitated with motor relearning program therapy, Neurological deficits and Motor functionin and Barthel Index of all patients were assessed before and after three days rehabilition. Results: The neurological deficit scores were reduced after rehabilition in the two groups.The differences were significant (p<0.01),The motor scores and BI were increased after rehabilition in the two groups.The differences were significant (p<0.05) ,the process of the neurological deficit scores in the rehabilition group were obviously superior to that in the control group(p<0.01), the process of scores on the motor and BI in the rehabilition group were obviously superior to that in the control group(p<0.05).Conclusion:Early rehabilitation with motor relearning program combining with leech and xueshuantong therapy on hemiplgics after stroke may obviously improve motor function,depressing the disability and increasing the quality of living.
