肾上腺皮质激素在SARS治疗中剂量及时机的探讨

The study of the dosage and the occasio of adrenal cortical hormone in usage of SARS

目的 :通过对 2 0例SARS患者病例的分析 ,探讨肾上腺皮质激素在SARS治疗中的应用 ,为SARS的治疗提供依据。方法 :按照中国疾病控制预防中心 4月 18日公布的诊断标准 ,选择 2 0例临床诊断病例 ,按照使用激素的初始剂量不同 ,将患者分组为亚冲击剂量组 (初始剂量≤ 16 0mg/d)和冲击剂量组 (初始剂量≥32 0mg/d) ,采用回顾性分析的方法 ,统计分析两组患者激素应用的过程 ,总激素用量 ,初次使用激素的时间 ,总病程 ,发热至体温正常的时间等指标。结果 :两组激素使用的时机 ,两组的病程 ,两组发热至体温正常的时间均无显著性差异 ,但是亚冲击剂量组使用的激素总量 (甲基强的松龙 )为 (40 0 4 0 1± 14 12 83)mg ,冲击剂量组使用的激素总量 (甲基强的松龙 )为 (5 6 12 0 1± 15 85 30 )mg ,冲击剂量组明显高于亚冲击剂量组 (P =0 0 2 8)。结论 :肾上腺皮质激素在SARS治疗中具有肯定的疗效 ,我们推荐亚冲击疗法。

Objective:To study the application of adrenal cortical hormone used in theatment of SARS,through analysis with 20 SARS cases,in order to provid to the theatment of SARS.Methods:20 clinically diagnosed SARS patients was enrolled in this study.The norm of diagnosis was published by China disease control center(CDC) in April 18,2003.The patiens were divided with the besub impact dosage group(the initial dosage ≤160mg/day)and the impact dosage group(the initial dosagemg≥320mg /day).Using the methed of retrospective analysis,we analysised the procedure of the application of the hormones,the dose of the hormones,the first time of the usage of the hormones,the course of the disease,the time from febrile to normal,and so on.Results:In those two groups,there was no significant difference in the occasion of usage of the hormones,the the course of the disease and the time from febrile to normal.But the dose of the hormones of the sub-impact dosage group was less than the impact dosage group′s( P =0 028).Conclusion:The adrenal cortical hormone can be used in SARS.And we recommended the sub implosion therapy.
