中国七城市老年高血压干预试验与脑卒中发病率、死亡率的研究

INTERVENTION TRIAL OF SENIOR CITIZEN WITH HYPERTENSION IN SEVEN CITIES OF CHINA AND THE STUDY OF THE INCIDENCE AND THE MOR-TALITY OF THE CEREBRAL VESSEL DISE ASE

本文报告了我国七城市（北京、上海、哈尔滨、长春、郑州、长沙、银川）９７５７名６０岁以上市区居民的高血压干预实验与脑卒中发病率、死亡率关系的前瞻性研究结果。干预组４５８９人；对照组５１６８人。对干预组人群实施多种形式的防治高血压知识的宣传教育并对高血压患者进行定期随访，合理饮食，药物治疗等干预措施，而对照组只观察变化，不采取干预措施。干预和观察时间三年。研究结果显示：１．干预组收缩压和舒张压干预后比干预前下降；而对照组却有所上升；２．干预组确诊高血压和临界高血压的比例，干预后比干预前均有下降，而对照组略有上升：３．脑卒中发病率随血压（收缩压或舒张压）的增高而上升；４．干预组脑卒中发病率死亡率都明显低于对照组。结果表明通过干预措施防治高血压是降低脑卒中二率的重要环节。

The incidence and the mortality of cerebral vessel disease (CVI)were studied in the study population consisting of 9757 senior citizensover the age of 60 in seven cities of china(Beijing, Shanghai,Haerbin,Zhengzhou, Changchun, Changsha, Yinchuan.) by intervention trial ofhypertension.A total of 9757 senior citizens were randomly placed inthe intervention trial group(groupIof 4589 persons)and the controlgroup(groupⅡ of 5168 persons).All subjects of groupIwere treatedwith various measures such as low-salt diet,pharmacotherapy ect.Follow-up were taken for the subjects with hypertension,The subjectsof group Ⅱ were only observed without giving interventional measures.The respective results were assessed after three year's observation andshowed that:1.There was a decline in both systolic and diastolic pres-sure in the subjects of groupI,on the contrary,there was a elevationin the subjects of group Ⅱ;2.A decline of the ratios of both diagnosedand critical hypertension was observed in groupⅠ,while a elevation ingroup Ⅱ;3.the mortalities of the stroke increased with the elevation ofthe blood pressure proportionally;4.It is showed a lower incidenceand mortality of the stroke in groupⅠas compared with control. Theabove results indicte that the interventional measures, prevention andtreatment of the hypertension are of importance in minimizing the inci-dence and mortality of the stroke
