化疗和免疫治疗对非小细胞肺癌患者淋巴细胞免疫功能的影响

EFFECTS OF CHEMOTHERAPY AND IMMUNOTHERAPY ON LYMPHOCYTE IMMUN-OFUNCTION IN PATIENTS WITH ADVANCED NON-SMALL CELL LUNG CANCER

目的 :观察化疗或化疗联合免疫治疗对非小细胞肺癌 (NSCL C)患者淋巴细胞免疫功能的影响及其与疗效的相关性。方法 :6 6例 NSCL C初治患者按治疗方式分成单纯化疗组 36例 ,化免组 (化疗联合 CD3AK细胞液治疗 ) 30例 ,在治疗前后检测患者淋巴细胞免疫的变化。以健康献血员外周血 T细胞亚群为正常对照。结果 :治疗前两个组 NSCL C患者的淋巴细胞免疫(CD3、CD4 和 CD4 / CD8比值及 NK计数 )均显著低于对照组 (P <0 .0 5 )。化疗组和化免组的有效率无显著性差异 (P >0 .0 5 )。化疗组中治疗有效病例 (CR+PR)淋巴细胞免疫较治疗前明显上升 (P <0 .0 5 ) ,但仍显著低于对照组 (P <0 .0 5 ) ,而稳定 (SD)和进展 (PD)病例的淋巴细胞免疫进一步下降 (P <0 .0 5 )。化免组中 CR+PR病例的细胞免疫明显改善 ,与对照组无显著性差异 (P >0 .0 5 ) ,SD病例的细胞免疫功能无明显变化 (P >0 .0 5 ) ,而 PD病例的细胞免疫仍显著下降 (P <0 .0 5 )。结论 :治疗有效病例的细胞免疫功能得到明显改善。联合 CD3AK治疗可以加快治疗有效病例的免疫功能恢复正常。SD、PD病例接受

Objective:To observe the effects of chemotherapy and immunotherapy on lymphocyte immunofunction in 66 patients with advanced non small cell lung cancer(NSCLC).Methods:36 patients treated with chemotherapy alone (C group), and 30 patients treated with chemotherapy combined with CD 3AK cells (CI group). T lymphocyte subgroup and NK cell count were tested before and after treatment.Result:T lymphocyte subgroup and NK cell count were significantly lower than that of control group before treatment(P<0.05). Response rates were 41.7% and 43 3%,respectively in the C group and the CI group (P>0 05). After therapy, CD 3,CD 4 and the ratio of CD 4/CD 8 and NK cell count were higher in improved(CR+PR) patients in C group (P<0 05), while CD 3,CD 4 and the ratio of CD 4/CD 8 and NK cell count were still lower than that of control group (P<0 05). In CI group, CD 3,CD 4 and the ratio of CD 4/CD 8 and NK cell count were higher in CR+PR patients (P<0 05), and there was no significant difference in cell immunofunction compared with control group after treatment (P>0 05).Conclusion:Effective therapy can improve lymphocyte immunofunction of the patients with NSCLC, especially chemotherapy combined with CD 3AK cells.
