以护士为基础、麻醉医师为督导的疼痛管理模式用于妇科术后镇痛

Nurse-based, Anesthesiologist-supervised pain management model in gynecologic surgical procedures

目的评价以护士为基础、以麻醉医师为督导的疼痛管理模式(NBASAPS)在临床术后疼痛管理中应用的成效。方法对实施该疼痛管理模式前后的788例术后病人进行开放性、前瞻性、非随机的观察性研究,分为两组,即实施APS前组(388例)和实施APS后组(400例)。观察镇痛效果。结果与实施APS前组比较,实施APS后组的术后镇痛效果指标(VAS值)显著降低(P<0.01);术后24h和48h的睡眠时间显著增加(P<0.01);住院时间显著缩短(P<0.01);病人总体满意度显著提高(P<0.01)。结论实施以护士为基础、以麻醉医师为督导的术后疼痛管理模式能显著提高术后镇痛效果。

Objective To evaluate the effectiveness of the nurse-based, anesthesiologist-supervised pain management model used in postoperative pain management.Methods 388 patients in the Pre-APS inception phase and 400 patients in the Post-APS inception phase were enrolled in an open, prospective, nonrandom,observational study.Results Compared with the Pre-APS Group,Post-APS's mean VAS was significant lower (P<0.01);Sleeping time within 24h after the operation was longer and the same within 48h(P<0.01); Hospitalization time was shorter than Pre-APS Group (P<0.01); patients satisfaction rate was higher in the Post-APS Group.Conclusion Nurse-based, anesthesiologist-supervised pain management model is a significant improvement in postoperative analgesia.
