Triple-phase spiral CT scanning in hepatic hemangiomas

目的 :评价肝血管瘤在螺旋CT动脉期、门脉期和延迟期增强扫描的特点 ,探讨螺旋CT对肝血管瘤的诊断价值。方法 :回顾性分析经螺旋CT增强检查的21例共28个血管瘤病灶的扫描资料 ,记录各病灶在动脉期、门脉期和延迟期的增强形态特点 ,比较病灶大小与各扫描期的关系。结果 :在动脉期有64 29 % (18/28)的病灶显示边缘结节状强化 ;在门脉期 ,该强化结节扩大 ,并向病灶中心扩展 ,其中16 67 % (3/18)呈现均匀的高密度 ;14 29 % (4/28)的病灶在动脉期无强化 ,门脉期为边缘结节状强化 ;10 71 %(3/28)的病灶在动脉期和门脉期强化明显 ,显示为均匀的高密度影 ;而7 14 % (2/28)的病灶在动脉期和门脉期均无强化 ,显示为均匀的低密度灶 ;3 57 % (1/28)的病灶在动脉期呈均匀高密度 ,门脉期为等密度。延迟扫描期 ,78 57 % (22/28)的病灶完全充填 ,显示为均匀的等密度或高密度影 ,其余21 43 % (6/28)部分充填。结论

Objective:To evaluate the enhancement imaging features of hepatic hemangiomas at arterial phase,portal venous phase and deˉlayed phase of spiral CTscanning,and to investigate the value of spiral scanning in the diagnosis of hepatic hemangiomas.Methods:Triple-phase spiral CTscanning of28hemangiomas in21cases was reviewed,the morphologic features and the correlation between the lesion size and the enˉhancement features had been studied.Results:64.29%(18/28)of hepatic hemangiomas had peripheral nodular potentialization at hepatic arterial phase(HAP),and centripetal oriented enlargement of the nodular regions at portal venous phase(PVP),among which16.17%(3/18)showed homogenenous high density at PVP;14.29%(4/28)of the lesions changed from lowattenuation at HAP to peripheral nodular enhancement at PVP;10.71%(3/28)of the lesions showed homogenenous potentialization at both HAP and PVP;7.14%(2/28)were of lowattenuation at both HAP and PVP;3.57%(1/28)changed from homogenenous high density at HAP to iso-density at PVP.In delayed phase,78.57%(22/28)of hepatic hemangiomas showed completely homogenenous patentialization or iso-density filling,other21.43%(6/28)presented part fillˉing.Conclusion:Triple-phase spiral CTscanning has an important value for the diagnosis of hepatic hemangiomas.
