经皮穿刺氩氦刀冷冻与肝动脉栓塞化疗联合应用治疗肝脏恶性肿瘤

Percutaneous cryoablation combined with TACE for liver carcinoma

目的 探讨经皮穿刺氩氦刀冷冻对已行肝动脉栓塞化疗 (TACE)的肝脏恶性肿瘤患者的可行性、安全性及有效性。方法 7例已行TACE的肝脏恶性肿瘤患者中 6例为原发性肝癌 ,1例为转移性肝肿瘤。CT导向下采用Seldinger法置入 11F自制标记导管鞘 ,用 3mm氩氦刀行经皮穿刺冷冻治疗。结果 7例 (10个病灶 )穿刺及冷冻均获得成功 ,无严重并发症发生。平均随访时间 7.5个月 ,部分缓解 (PR) 4例 ,无变化 (NC) 2例 ,进展 (PD) 1例。结论 经皮穿刺氩氦刀冷冻与TACE联合应用对肝脏恶性肿瘤是一种安全有效的治疗方法 ,二者联合应用可弥补两种疗法可能存在的不足。

Objective To explore the feasibility, effectiveness and safety of percutaneous cryoablation combined with transcatheter arterial chemoembolization (TACE) in the treatment of liver carcinoma. Methods Percutaneous cryoablation was performed in 7 patients(6 primary liver cancer and 1 metastatic liver cancer), which were treated with preoperational TACE. With CT guidance, 11-F catheter sheath and 3-mm cryosurgery system were employed.Results The therapy was successfully performed in 7 patients ( 10 regions). No serious complications occurred. The average follow up was 7.5 months, and PR 4 cases, NR 2 cases, PD 1 case. Conclusions Percutaneous cryoablation for liver carcinoma after TACE is safe and effective,it may be a useful supplement for each method alone.
