摘要
目的:探讨高血压前期人群大动脉顺应性的变化。方法:选择年龄相似血压正常的健康者及没有任何靶器官损害的Ⅰ-Ⅱ级高血压患者147例,分为三组:正常对照组54人;高血压前期组57人;高血压组36人。应用脉搏波速度(PWV)测定仪对所有入选者进行同时测定颈动脉-桡动脉(C-RPWV),颈动脉-股动脉的脉搏波速度(C-FPWV),及血压、身高、体重、体重指数(BMI)、腰围、臀围、腰臀围比值、心率(HR)等。结果:C-FPWV、C-RPWV在正常组、高血压前期组、高血压组三组中分别为(8.82±1.60)m/s、(8.77±1.07)m/s、;(9.66±1.60)m/s、(9.78±1.48)m/s;(10.07±1.45)m/s、(10.51±1.64)m/s。方差分析示,三组间各因素比较除心率外均存在统计学差异。两组间的S-N-K检验示:高血压前期组的身高、体重、BMI、腰臀围比值,C-R PWV及C-FPWV均大于正常组,P<0.05;而年龄、脉压(PP)、心率无统计学差异,P>0.05。高血压前期组与高血压组的比较,C-R PWV有明显的统计学差异,P<0.05。血压正常的健康人(高血压前期组和正常对照组)的PWV与各因素的简单相关(Pearson)分析示:C-FPWV、C-RPWV都与年龄、收缩压(SBP)、舒张压(DBP)、身高、臀围、HR相关;C-FPWV还与PP相关。C-FPWV、C-RPWV多因素逐步线性回归分析示:C-FPWV与年龄、SBP、身高、体重。
Objective: To investigate the changes of large arterial compliance in prehypertensive people. Methods: The 147 persons including appearance healthy and Ⅰ-Ⅱ hypertensives without any damaged target organ were divided into three groups: 54 nor-motensive persons, 57 prehypertensive persons and 36 hypertensive patients. Their pulse wave velocities (PWV) were measured by the Complior SP including carotid - radial ( C - RPWV) and carotid - femoral (C - FPWV). The other parameters involved in age, systolic blood pressure (SBP) , diastolic blood pressure (DBP) , pulse pressure (PP) , weight, height, body mass index (BMI) , waist a-round, buttocks around, ratio of waist around to buttocks around and heart rates (HR). Results: C - FPWV, C - RPWV of normoten-sive group, prehypertensive group, hypertensive group are (8. 82±1.60) m/s, (8.77± 1.07) m/s; (9.66±1.60) m/s, (9.78±1. 48) m/s; and (10.07±1.45) m/s ,(10.51±1.64) m/s. One - way ANOVA analysis showed that most of factors have statistical significances between the three groups instead of HR. S - N - K test between two groups showed that weight, height, BMI, their ratio of waist and buttocks around, C - RPWV and C - FPWV in prehypertensive group were higher than those of normotensive group, P < 0. 05; and its C - RPWV was lower than those of hypertensive group, P <0.05. In normal people (normotensive and prehypertensive group) , C -FPWV and C - RPWV had correlations with age, SBP, C - RPWV and HR, and C - FPWV associated with PP, P < 0.05. The multiple factors regressive analysis showed that C - FPWV had the intimacy with age, SBP, PP and weight; C - RPWV had the intimacy with DBP and HR. Conclusions: There is the change of large arterial compliance in prehypertensive patients. Arterial compliance decreases with the increase of age, blood pressure, and heart rate in nonhypertensive patients.
