妇科腔镜手术的肠道准备方法探讨

INTESTINAL TRACT PREPARATION FOR GYNECOLOGICAL OPERATIONS BY LAPAROSCOPE

目的:探讨采用甘露醇进行妇科腔镜手术肠道准备的可行性。方法:对400例妇科腔镜择期手术患者,随机分为3组,A组280例,采用口服甘露醇同时加服抗生素;B组60例,采用口服甘露醇;C组60例,采用传统灌肠方法。并对临床清肠效果及副反应进行比较。结果:清肠效果达到良好者,A组270例(95.8％),B组56例(93.1％),C组55例(92.5％)。手术平均时间(min):A组120.3±28.4,B组128.2±28.7,C组125±20.3。术后肛门排气时间(h)分别为:A组24±10.5,B组25.1±12.7,C组30±18.3。3组术后并发症无显著差异(P>0.05)。结论:采用甘露醇进行肠道准备简单、安全、有效,尤其加服抗生素,可抑制肠腔细菌生长,有推广应用价值。

Objective: To explore the feasibility of mannitol used for intestinal tract preparation for gynecological.laparo-scopic operation. Method: 400 patients who received laparoscopic operations were divided into three groups at random. Group A, 280 patients took mannitol plus antibiotics orally; Group B, 60 patients took mannital only; Group C, 60 patients conducted emema. The results among the groups were compared. Results :270 patients (95.8%) in Group A, 56 (93.1%) in Group B and 55 (92.5% ) in Group C received satisfied results. Average time of operation was 120.3 + 28.4, 128 + 28. 7 and 125 + 20.3 for Group A,B and C respectively. Time to anal exsufflation postoperatively was 24 ± 10. 5 hours, 25. 1 ± 12. 7 hours and 30 ±18. 3 hours respectively. There was no significantly difference for incidence of complication among the groups (P >0.05). Conclusion: Mannitol plus antibiotics are a safe, effective and simple method for intestinal tract preparation of gynecological operations.
