头状骨带蒂移位与血管蒂骨瓣植入治疗月骨无菌坏死

Capitate Bone With Pedicle Transposition and Bone Flap with Vascular Pedicle Implantation for Treatment of Lunate Asepsis Necrosis.

目的 报道带骨间前动脉背侧支血管蒂的头状骨移位术与以桡动脉茎突返支为蒂的桡骨瓣植入术联合应用治疗月骨晚期缺血性坏死的结果.方法 对46例患者行坏死月骨摘除头状骨带蒂移位后以血管蒂骨瓣植入填塞头状骨空隙.结果 术后随访16～48个月,平均21.5个月,43例腕痛完全消失,3例明显缓解,握力平均达健侧80%,腕关节活动范围达健侧的75%.结论 两种术式的联合应用对Ⅲ、Ⅳ期月骨缺血性坏死疗效显著.

ve To report the effect of treating lunar bone necrosis by capitate bone with the dorsal branch of the anterior interosseous artery transposition and radius flap with styloid recurrent artery of the radial artery implantation. Methods 46 cases were treated by bone flap with vascular pedicle implantation into the space of capitate bone, after necrotic lunar bone and capitate bone with pedicle were removdse out. Results All cases were followed up for 16 -48 months, with an average of 21. 5 months. Relief of pain was obtained in 43cases. Pain was remitted obviously in 3 case, The grip strength was 80% of the mormal side. The activity range of the wrist was 75 % of the mormal side. Conclusions It is an effective method to treat kienbock 's disease of stage Ⅲ and Ⅳ that the two operation are applied commonly.
