从首钢社区十年来人群血压及血清胆固醇含量的变化看脑卒中的一级预防

Analysing primary prevention of cerebral infarction with the change both blood pressure and serum cholesterol of crowd in 10 years in the region of Capital iron & steel Co

目的：为了解首钢社区工作十年间的总体效果。方法：为脑卒中一级预防提供依据。首钢心血管病防治区对一组中年样本人群进行危险因素调查，十年后进行队列复查，期间对防治区人群进行健康教育及高血压病人管理。基线和复查人群平均血压分别为１６．６／１０．７ｋＰａ，１６．５／１０．７ｋＰａ收缩压（ＳＢＰ）下降１％，舒张压（ＤＢＰ）无变化。同时该区脑卒中标化发病率为９７．９３／１０万一致。结果：提示降低人群平均血压值，即或是较少的数也能防止脑卒中发病的过度增长；人群血清总胆固醇（ＴＣ）均值基线和复查分别为４．８４ｍｍｏｌ／Ｌ和４．７８ｍｍｏｌ／Ｌ，后期ＴＣ总体水平略下降，而脑卒中标化发病率变化不大，且男性ＴＣ水平与日后发生脑出血的危险呈逆向关系，提示宜把ＴＣ调整到适度水平为妥。结论：通过降低人群平均血压值，即或是很小的数，也能防止脑卒中发病过度增长。

in order to know effect of health work in 10 years in the region of Capital rion & steel Co,we study on the risk factors in heart and blood vessel disease of middle age persons.During 10 years we made health education and hypertesion administration. The persons’ blood pressure average in investigations both Basic line and 10 year end are 16.6/10.7kPa and 16.5/10.7kPa severally.SBP lowered 1%,DBP did not change.The incidence rate of cerebral infaction in the region is 97.93/10万.The persons’ serum cholesterol average in investigation both basic line and 10 year end are 4.84mmol/L and 4.78mmol/L severally.The relation both men TC quantity and the risk of cerebral hemorrhage is opposed to each other suggests suitable TC quantity is good. The result shows:Blood pressure lowered,even a little,can prevent excessive inciease of cerebral infarction.The result supports.The best primary prevention method of cerebral infarction is to lower persons blood pressure average.
