西宁地区805例脐带绕颈胎儿临床分析

CLINICAL ANALYSIS ON 805 CASES OF FFTAL UMBILICAL CORD AROUND NECK AT XINING AREA

本文分析了我院 1997年 1月至 1998年 12月 2年间分娩的 3 3 2 1例资料。发现脐带绕颈者 80 5例 ,发生率为 2 4 2 3 % ,明显高于国内平原地区资料 ( 17 4 3 % ) 〔2〕。本文资料显示脐带绕颈一周平均长度为 5 6 2± 2 11厘米 ;2周者为 64 8± 3 4 4厘米 ;3周以上者为 90 2± 3 4 9厘米 ;最长者为 110厘米 ,由此可见 ,脐带长度与绕颈有明显的关系。脐带绕颈以初产妇较经产妇发生率高 ,分别为 2 5 77% ( 773 / 3 0 0 0 ) ,9 97% ( 3 2 / 3 2 1) ,差异有显著性 (P <0 0 1)。在 80 5例脐带绕颈者中 ,发生死胎死产 4 3例 ,病死率为 5 3 4 %。因胎儿宫内窘迫而施行剖宫产者 3 5 65 % ,而无脐带绕颈者剖宫产率为 16 89%

The data of 3321 cases of babies born in our hospital from Jan 1997 to Feb 1998 was analyzed.Among them 805 cases(24.23%)were occurred fetal umbilical cords around neck and the occurring rate was significantly higher than that of in the plain.The average length of fetal umbilical cords around nick a circuit was 56±2.11cm,two circuits 64.8±3.44cm,three circuits or more than that was 90.2±3.49cm.The longest was 110cm.It showed that the length of umbilical cord was closely related with the occurrence of it winding around the nick.The incidence was higher in primipavas(25.77%)than that of multigravidas(9.97%)and showed a significant difference(P<0.01).There were 43 cases of stillbirth and stillborn foetus among 805 cases of umbilical cords winding around the nick and the mortality was 5.34%.The Caesarean birth was performed in 35.65% of foetus suffering fetal distress.
