机械通气治疗急性呼吸窘迫综合征27例分析

APPLICATION OF MECHANICAL VENTILATION THERAPY IN ARDS

目的 对急性呼吸窘迫综合征 (ARDS)行机械通气治疗进行观察分析。方法 对 2 7例ARDS患者采用同步间歇指令通气加压力支持 (SIMV +PSV) ,应用较低潮气量 ( 6～ 10ml/kg) ,吸入氧浓度 (FiO2 ) <5 0 % ,PEEP≤15cmH2 O。结果 2 3例存活 (占 85 .19% ) ,4例死亡 (占 14 .81% )。结论 造成ARDS患者死亡的主要原因为并发多器官功能障碍综合征 (MODS)。早期控制原发病是ARDS治疗的关键 。

Objective:Observation of mechanical ventilation therapy in acute respiratory distress syndrome (ARDS).Methods:27 cases of ARDS were adopted synchronized intermittent mandatory ventilation and pressure support ventilation (SIMV+PSV),low tidal volume (6～10ml/kg) was used,FiO<50% and positive end expiratory pressure (PEEP)<15cmH 2O were used.Result:23 cases were survived (85.19%),4 cases were dead (14.81%).Conclusion:The main causes of death were ARDS complicated with MODS (Multiple Organ Dysfunction Syndrome),control primary disease is key for ARDS treatment,mechanical ventilation therapy is an important method for treatment process.
