呼吸机相关性肺损伤17例回顾性分析

A retrospective study of 17 cases of ventilator-associated lung injury

目的 提高对呼吸机相关性肺损伤(VALI)的认识。方法 对1986年～2001年行机械通气(MV)过程中发生VALI的17例患者进行回顾性分析。结果 17例VALI中包括气胸5例,系统性气栓塞4例,急性肺损伤4例,弥漫性肺损伤4例。结论 呼吸机因素和患者本身因素是发生VALI的重要原因;制定更为合理的个体化通气策略及其它措施是提高MV治疗价值的关键。

Objective To study the clinical characteristics of ventilator-associated lung injury (VALI) .Methods 17 cases of VALI during mechanic ventilation (MV) from 1986 to 2001were analysed retrospectively.Results In 17 cases,pneumothorax occurred in 5 cases,systemic gas embolism in 4 cases, ALI in 4 cases,and diffuse lung injury in 4 cases.Conclusion Both positive ventilation and patient's underlying conditions are involved in the pathogenesis of VALI .A individual strategy of ventilation is critical to prevent VLAI.
