肝硬化患者胰岛素抵抗的另一可能机制初近探

摘要 肝硬化患行普遍存在高胰岛素血症(Ins)和胰岛素抵抗,胰岛素样生长因子Ⅰ(IGF-Ⅰ)的合成受Ins的影响。为探ICF—Ⅰ及其结合蛋门-3(IGFBP-3)与肝硬化患者胰岛素抵抗的关系,本文应用放射免疫法检测了65例肝硬化患者血清IGF-I和IGFBP-3及Ins水平。结果表明:肝硬化患者血清IGF-Ⅰ和IGFBP—3水平下降(100.49±38.11ng/ml对352.55±153.61ng/ml,P<0.001;45.02±16.21nm01/L对114.50+27.09nm01/L,P<0.001),而Ins水平明显升高(30.38±13.21对5.69±3.47,P<0.001),IGF-Ⅰ、IGFBP-3与His呈负相关(P<0.01)。结论:肝硬化患者存在胰岛素抵抗,IGF-Ⅰ、IGFBP-3可能与肝硬化患者的胰岛素抵抗有关。 Most of the cirrhotic patients are associated with high insulin (Ins) levels and insulinresistance. The syntheis of insulin-like growth factor Ⅰ(IGF-Ⅰ) was modulated by insulin. The aim of this study was analyze the correlation between IGF-Ⅰ, IGFBP-3 and insulin resistance of cirrhosis. METHODS: Sera were obtained from 65 patients with cirrhosis and from 30 controls after a 12h fast. Serum levels of insulin, IGF-Ⅰ and IGFBP-3 were measured by radioimmunoassay. RESULTS: Serum IGF-Ⅰ and IGFBP-3 levels were reduced m cirrhosis but not in controls (100.49_+38.ling/mi vs 352.55-+153.6lng/mi, P<0.001; 45.02_+16.21nmol/L vs114.50 _+27.09nmol/L, P<0.001 ). Insulin level significantly increased in patients with cirrhosis (30.38 4-13.21 vs 5.69 4-3.47, P<0.001). There was significant negative correlation betweenIGF-Ⅰ, IGFBP-3 and insulin (P<0.01). CONCLUSIONS: There was insulin resistance in cirrhosis. IGF-Ⅰ~. and IGFBP-3 levels were inversely correlated with insulin level incirrhosis. Thereforce IGF-Ⅰ and IGFBP-3 levels might correlate with insulin resistance.
