Nd:YAG激光在眼球前节疾病的临床应用

Application of Q-switched Nd:YAG Laser in Treatment of Anterior Segment Eye Diseases

应用Q开关Nd : YAG激光治疗2160例共2266只眼球前节疾病获得成功。其中男907例,女1253例,年龄2～88岁,平均45岁;观察3个月至5年。能量选择及脉冲数系根据击射组织的具体情况而定,重复击射最多达4次,每次相隔3～7天。部位包括:晶体、虹膜、瞳孔、玻璃体以及前房角等五处,病例情况分别为(1)膜样白内障;(2)晶体前后囊膜切开;(3)白内障促成熟;(4)各类型青光眼;(5)虹膜囊肿;(6)瞳孔移位及前后粘连;(7)先天性残存瞳孔膜;(8)玻璃体疝;(9)玻璃体机化及条索切开;(10)重开青光眼滤过术后闭塞性的瘘口;(11)松解结膜下尼龙线。同时报告一典型病例说明重新开放青光眼术后闭塞的瘘口,避免了第三次抗青光眼手术的风险。

The Q-switched neodymium:YAG laser was successfully used in treatment of anterior segment eye diseases in 2160 patients (2266 eyes). The power and amount of pulses were selected according to the actual conditions of the lesions beingshot. The areas shot included the lens, iris, pupil, vitreum and angle of the anterior chamber The cases were classified into the follwing groups: (1) secondary membranous cataract, (2) anterior and posterior capsulotomy of the lens, (3) acceleration of cataract maturatio, (4) various types of glaucoma, (5) cysts of the iris, (6) dislocation and adhesion of the pupil, (7) congenital remnant membrane of the pupil, (8) vitreous hernia, (9) vitreolysis, (10) reopening of closcd filtering fistula after glaucomatous filtration, and (11) undoing nylon thread under the conjunctivao Details of treatment using Nd:YAG laser in a typical case with reopening of closed filtering fistula after an operation on glaucoma were presented, by which the risk of the third antiglaucoma operation was avoided.
