腰椎滑脱症的手术治疗

Surgical treatment of the lumbar spondylolisthesis

自１９８５年６月～１９９１年１０月对４９例腰椎滑脱伴有神经症状者进行了手术治疗。对２８例腰椎滑脱小于２５％者，给予椎管、根管扩大减压后行棘突间“Ｈ”型植骨和横突间植骨融合术。２１例滑脱大于２５％者，除采用扩大性减压外，同时行Ｓｔｅｆｆｅｅ槽式钢板复位内固定加横突间植骨融合术。经过３年６个月随访。优良率为８５．６％，有效率９５．８％。作者对外科治疗的选择作了讨论。

From 1985 to 1991, 28 cases of olisthe less than 25% were treated with interspinal 'H', type and intertransverse bone grafting and fusion after removing mobilized spinous process and vertebral lamina, enlarging decompression of vertebral and root canal. 21 cases of olisthe more than 25% were treated by reduction and internal fixation with Steffee' B intertransverse bonegrafting and fusion. After followed up for 3 years and 6 months, good and excellent rate were 85. 6% and effective rate was 95. 8% according to wiltse method. The Choice of surgical treatment were discussed.
