抑制胃酸分泌对正常人胃肠激素释放及迷走神经功能影响的研究

The influence of acid inhibition on the release of gut hormones and vagal nerve function in healthy subjects

目的探讨奥美拉唑抑制胃酸分泌前后正常人盲肠激素释放及边走神经功能变化。方法采用放克和生物测定法，对用药前后基础和餐后不同时间血浆生长抑素、血管活性肠肽、胰多肽和胆囊收缩素浓度进行测定和比较。结果抑酸对血浆生长抑素、血管活性肠肽和胆囊收缩素释放影响不大；餐后胰多肽释放明显减少，迷走神经张力明显下降。结论奥美拉唑抑酸治疗可对胃肠激囊释放产生一定影响；速走神经张力降低可能是导致接受抑酸治疗患者胃及胆囊动力低下的原因。

Purpose To assess the influence of acid inhibition on the release of gut hormones and vagal nerve function. Methods Seven healthy subjects participated in this 7-day study. Venous blood samples were collected at fasting state、 15、 30、 60 and 120min postpran dially before and on the seventh day of 20mg omeprazole daily. SST、VIP and PP were measured by RIA and CCK by Bioassay. Results Acid inhibition had no significant influence on basal and postprandial secretion of plasma SST、VIP and CCK;Postprandial release of PP was significantly decreased,suggesting that the vagal nerve tone was significantly lowered after omeprazole. Conclusions Acid in hibition by omeprazole might affect the fasting or postprandial secretion of certain gut hormones,and produce unfavorable in fluence on the vagal nerve.which may result in gastric and gallbladder hypo-motility. Combined treatment with pro-kinetic agents is recommended for the patients in need of long term acid inhibition therapy.
