细胞凋亡及C—myc蛋白表达与胃癌发生的关系

SIGNIFICANCE OF APOPTOSIS AND EXPRESSION OF C-myc IN GASTRIC PRE-CARCINOMA AND CANCER

目的:了解胃癌及癌前病变的细胞凋亡与 C-myc 表达的关系。方法:采用原位末端标记法(TUNEL)检测细胞凋亡。采用 LSAB 免疫组化法检测 C-myc 蛋白表达。结果慢性活动性胃炎、胃溃疡、轻度不典型增生、重度不典型增生、早期胃癌、进展期胃癌的凋亡指数分别为16.8%±12.3%、24.1%±20.0%、19.31%±6.4%、15.7%±15.2%、10.1%±9.1%和6.3%±6.0%,进展期胃癌显著低于早期胃癌和不典型增生(p<0.05)。而 C-myc 蛋白的阳性表达率分别为15.6%、20.7%、35.3%、50.0%、66.7%。胃癌和重度不典型增生的阳性表达率明显高于胃溃疡和活动性胃炎(p<0.01),进展期胃癌明显高于轻度不典型增生(p<0.05)。在胃癌患者中,C-myc 阳性组与阴性组凋亡指数分别为8.2%±6.0%和3.5%±2.1%,两组比较有显著差异(p<0.05)。结论:在胃粘膜不典型增生、早期胃癌到进展期胃癌的发展中,细胞凋亡逐渐受到抑制。其 C-myc 蛋白的阳性表达率有逐渐升高的趋势.尤胃癌和重度不典型增生有较高的阳性表达率。C-myc 阳性的胃癌有更高的凋亡指数。

Objective:To clarify the significance of apoptosis and expression of C-myc in gastric pre-car- cinoma and cancers.Methods;TUNEL method was used to detect apoptosis.Immunohistochemical analysis was used to detect the expressions of C-myc protein.Results:Apoptosis index of chronic active gastritis, gastric ulcer、light atypical hyperplasia、severe、atypical hyperplasia were found 16.8％±12.3％、24.1％± 20.0％、19.31％±6.4％、15.7％±15.2％、10.1％±9.1％ and 6.3％±6.0％ respectively and positive ex- pression rates of C-myc were 15.6％、20.7％、35.3％、50.0％、50.0％ and 66.7％ respectively.Apoptotic indexes were significantly lower in progress gastric carcinomas than that in early gastric carcinomas and atypical hyperplasia(p<0.05)C-myc positive rates were significanlty higher in cancers and severe atypical hyperplasia than that in ulcer and active gastritis(p<0.01),and were significantly higher in progress can- cers than that in light atypical hyperplasia(P<0.05).Apoptosis index active was lower in the negative group 3.5％±2.1％ than that in the positive one(8.2％±6.0％).Conclusion:From atypical hyperplasia, early gastric carcinomas to progress gastric carcioma the formation of gastric carcinomas are closely related to apoptotic indexes and the expression of C—myc.
