补肾方药治疗骨质疏松的实验研究与临床观察 被引量:31

Experimental study and clinical observation of osteoporosis treatment with kidney replenishing herbs

摘要 根据中医“肾主骨”的理论,运用生物化学的理论和方法,从选用补肾方药入手,对骨质疏松进行了实验研究和临床观察,为中西医结合防治骨质疏松提供了理论依据和有效方药。实验研究,选用Wistar雌性大鼠,用地塞米松制造骨质疏松病理模型。随机分为正常对照组、病理对照组和中药治疗组,12周观察变化。中药治疗组证明:骨密度、组织形态学变化、骨灰重与病理组比较均有明显改善;血中雌二醇(E2)、1,25-(OH)2-D3呈正相关,与PGE2呈负相关;可促进肠钙的吸收,增加CaBP-D9kmRNA的生成;尿羟脯氨酸明显降低。临床观察,确诊为原发性骨质疏松病人90例,随机分为补肾中药组、降钙素组和阿仑膦酸钠组进行对照研究,疗程8周和12周,继续观察1年。结果揭示,BMD无明显差异,血钙提高,PTH下降,CT和E2提高,综合疗效评定A组为90%,B组为83%,C组为73.38% Using the method of biochemistry, we engaged in the experimental study and clinical observation of the osteoporosis treatment with kidney replenishing herbs on the basis of the theory of traditional Chinese medicine“the condition of the kidney determines the condition of the bone”so as to provide scientific basis and efficacious recipe for osteoporosis prevention and treatment. Experimental study: Wistar female rats (aged four months) were used and divided randomily : group 1, normal control; group 2, rat models with osteoporosis, which was induced by dexamethasone(DXM) given twice a week; group 3, rats given DXM and treated with kidney repenlishing herbs. 8 weeks and 12 weeks later, the change of these rats was observed. Results: The bone mineral density, bone ash weight, estrogen, 1, 25-(OH)2-D3 intestinal Ca(+2) absorption and CaBP-D9k mRNA were increased, while PEG 2 and urine hydroproline were declined in herbs treated rats compared with those of osteoporoticrats. Clinical observation: According to bone mineral density and clinical symptoms, 90 cases were diagnosed as hasing primary osteoporosis. These patients were divided randomly into 3 groups: group 1; treated with kidney replenishing herbs (KangCu Song granules) ;group 2,treated with calcitonin; group 3, treated with alendronate. After eight weeks ,we observed and compared the biochemical indexes with those before treatment. Results: The bonewineral densith increased in all treated patients and there was no remarkable difference. Te serum Ca(2+) was higher than that before treatment in group 1. CT and estrogen were higher, PTH were lower than those before treatment in group 1. According to the improvement of the bone mineral density, symptoms and biochemical indexes, the effective rates in groups 1,2 and 3 were 90%, 83% and 73. 38%, respectively. We found that the shi replenikidneyng herbs could improve bone matablism, remedy hormone imbalance and treat osteoporosis without any side effect.
