逆行股前外侧岛状皮瓣的解剖学基础和临床应用

Anatomic basis and clinical application of reversed anterolateral femoral island flap

在以旋股外侧动脉降支为蒂的股前外侧皮瓣解剖学研究基础上,为配合逆行股前外侧皮瓣移植修复膝关节周围软组织缺损,在10侧灌注乳胶标本和8侧下肢动脉铸型标本上,解剖观察了旋股外侧动脉降支与膝关节周围动脉的吻合;降支末端与膝上外动脉和其他膝周动脉有吻合。依据解剖基础,应用逆行岛状皮瓣或肌皮瓣修复14例膝周围软组织缺损,皮瓣全部成活,疗效良好。文中讨论了皮瓣设计、切取方法和优缺点。

On the basis of anatomical study of anterolateral femoral flaps,which is ped-icled with the descending branch of lateral circumflex femoral artery,the anast-omoses of the descending branch of lateral circumflex femoral artery with thearteries surrounding knee joint were studied in 10 latex injection and 8 arterialcast specimens of cadaveric lower extremities,for the design of reversed anterol-ateral femoral island flaps in repairing the soft tissue defect in the knee joint region.It is observed that the distal end of the descending branch anastomosiswith the lateral superior genicular artery and with other arteries around the kn-ee joint.14 cases of soft tissue defects in the knee joint region have been treatedwith reversed anterolateral femoral island flaps or musculocutaneous flaps.Flapsin all the cases survived well.The surgical design,operative technique,advanta-ges and disadvantages of the flap were discussed.
