摘要
本文报告了二氢埃托啡(DHE)临床治疗38例海洛因成瘾者的药效学研究,发现CINA(Clinical Institute Narcotic Assessment)量表的变化分值(可反映海洛因成瘾者的身体依赖性的程度与二氢埃托啡的48h舌下含服维持剂量有很强的相关性,其相关系数为0.8026;通过回归分析,得出CINA量表的变化分值与二氢埃托啡的48h舌下含服维持剂量的线性回归方程:DHE剂量(μg/h)=1.6309CINA+6.4554。剩余平方和与离差平方和之比为0.2964;通过曲线拟合所得的幂函数曲线方程为:DHE剂量(ug/h)=CINA^(0.0276)+22.0821,剩余平方和与离差平方和之比为0.2794,拟合效果有所改善;通过曲线拟合所得的三次多项式方程为:DHE剂量(μg/h)=0.0003CINA^3-0.0005CINA^2+0.6163CINA+24.9261,剩余平方和与离差平方和之比降低到0.2695,拟合效果进一步提高。由于CINA变化分值可反映海洛因成瘾者的身体依赖程度,根据海洛因成瘾者的身体依赖程度我们可由CINA分值由上述公式得到二氢埃托啡的48h舌下含服维持量。为了便于临床计算,建议使用线性回归方程计算二氢埃托啡舌下含服维持剂量。
This paper reported pharmacodynamics of DHE in clinical detoxification of 38 heroinaddicts. The correlation coefficient is 0.8026 between CINA (Clinical Institute Narcotic Assessment) Scalescore and the maintenance DHE dose sublingully. The maintenance dose used in the treatment of individualpatients was regressed on CINA score. The simple regression equeation was: DHE dose(microgram / hour)=1.6309 CINA Score + 6.4554. Further analysis indicated that the inclusion of the exponential termimproved prediction of the DHE dose by 2.2% (thus, a total of 72.06%) of the variance. The regressionequation could be given as: DHE dose(microgram / hour)=CINA^(0. 0276) + 22.0822. The polynomial termimprove prediction of the DHE dose by 3.1% (thus, a total of 73.05% ) of the variance. the regression e-quation could be given as: DHE dose(microgram / hour)=0.0003CINA^3 + 0. 0005CINA^2 + 0.6164CINA +24.9262. In order to determine maintenance DHE dose easily, we recommend the simple regression equa-tion: DHE dose(microgram / hour)=1.6309 CINA Score + 6.4554 for clinical use.
