小儿皮肤毛细血管瘤自然消退的演变规律及机理探讨

本文观察了90例小儿皮肤毛细血管瘤的组织形态,其中消退期64例的主要组织学特点是瘤组织的主质消退,间质增生,AgNOR和肥大细胞计数均低于增生期(P<0.01)。消退的形式有两型:一型是血管内皮细胞和毛细血管萎缩,管腔闭合、消失,纤维组织增生;另一型是内皮细胞和毛细血管数量减少,脂肪组织增生浸润。在Ⅰ期消退中。两种形式消退的发生率及量无明显差异。在Ⅱ、Ⅲ期消退中90%(27/30)是以第二种形式消退为主。消退率和消退范围均与病程成正比。

A histological study was carried out in 90 cases of childhood cutaneous capillary hemangiomas. 64 cases of the involuting phase tumors were characterized by parenehymatous regression and interstitial proliferation. The count of AgNOR and mast cells in the involuting phase was significantly lower than that in the proliferating phase (P<0. 01). There were two types of regression. One showes endothelial and capillary atrophy, vascular lumen atresia and disappear, fibrous tissue proliferation. The other exhibits the decreased endotheliocytes and capillaries, fatty tissue proliferation and infiltration. In regression of phase Ⅰ, incidence and quantity of the two types of regression were similar. In regression of phases Ⅱ and Ⅲ, the second type of regression predominates, accounting for overwhelming majority (90%)The rate and the extent of the regressions were in the direct ratio to the course of lesion.
