术中超声监视下尿道内置入镍钛形状记忆合金网状支架治疗前列腺增生症

摘要 从1994年2月至1997年5月,采用术中超声监视下尿道内置入镍钛形状记忆合金网状支架治疗高危前列腺增生症合并尿潴留54例,疗效满意。作者认为经直肠超声监视较经腹壁超声监视更清晰。采用经直肠超声观察尿道口,对单纯中叶增生使尿道内口前移,或单纯一例叶增生使尿道内口侧移者,应选用特制支架。测量前列腺尿道的长度,以经直肠超声法最为精确,经腹壁超声法及膀胱镜下输尿管导管测量皆有较大的误差。采用膀胱镜式置入器作支架置入缺点较多,专利支架种植器能迅速无误地将支架植入选定部位,操作更为简便。 Fifty four cases of urinary retention caused by high risk benign prostatic hyperplasia were treated with intraoperative ultrasonic monitoring intraurethral implanting Nickel - Titanium Shape - memory alloy net stem from Feb 1994 to May 1997. The results were satisfactory. The present study showed that the intraoperative monitor using transrectal ultrasound was more clear than using transabdominal ultrasound. It is important to observe internal ostium of the urethra in using transrectal ultrasound for selecting proper cases. Special stent should be applied in case of internal ostium of urethra was displaced forward due to middle lobe hypertrophy of the prostate and to left or right caused by an enlarged lateral lobe. The transrectal ultrasound method for measure the length of prostate urethra is more accurate than others. The patent implantor is better than others.
