尿淀粉酶测定监测猫移植胰腺的排斥反应 被引量:1

摘要 25只猫接受全胰十二指肠移植手术。A组8只猫,胰腺引流方式采用肠肠吻合,术后5只猫未接受任何抗免疫治疗,另外3只地塞米松肌肉注射。该组平均生存12d,死亡与胰腺坏死有关。B组17只猫,行十二指肠膀胱吻合。5只地塞米松肌肉注射。其余口服环孢霉素A。平均生存17d。9只猫有程度不等的胰腺坏死,其余8只胰腺组织学结构大致正常。术后所有实验猫血、尿淀粉酶均升高。尿淀粉酶升高的幅度差异很大,A组升高6倍,B组升高80倍。其差额可以反应移植胰腺的功能状态。胰腺发生坏死后,尿淀粉酶含量明显降低。胰腺没有坏死,尿淀粉酶仍居高不下。实验结果表明通过胰腺引流膀胱的术式,尿淀粉酶的降低与移植胰腺的坏死及功能丧失有明显关系。胰腺坏死及功能丧失是排斥反应损伤的结果。因此尿淀粉酶的测定可以作为监测移植胰腺排斥反应的发生与强度的重要手段。
