类似Ⅰ级星形细胞瘤的单纯疱疹病毒性脑炎的CT诊断

CT Diagnosis of HSV Ⅰ Type Encephalitis Simulating Ⅰ-Grade Stellate Cell Tumor

目的 :分析类似Ⅰ级星形细胞瘤的单纯疱疹病毒性 (HSV)Ⅰ型脑炎的CT表现 ,归纳总结其特征性改变 ,提高HSVⅠ型脑炎CT诊断的依据。方法 :7例经脑脊液PCR证实、2例经手术病理证实的HSV脑炎患者。采用SHIMADZUSCT -70 0 0TH扫描系统 ,全部病例行平扫后再行增强扫描。结果 :9例中单侧 6例、双侧 3例 7处病灶 ,总计 13处病灶 ;球形、类球形 7处 ,片状不规则形 6处 ;增强扫描 5处病灶有不同形式的强化 ,8处病灶无明显强化。结论 :多发性、均等低密度病灶 ;范围较大者所产生占位效应相对较轻 ;增强后无强化表现等是HSVⅠ型脑炎的特征。

Objective: To analize the CT manifestations, and provide the CT diagnostic criteia for herpes encephalitis simplex. Materials and Methods: Nine patients with herpes encephalitis simplex proved by cerebrospinal fluid PCR technique in seven cases and by pathology in two cases, were carried out with plain and enhanced CT scans on SHIMADZU SCT-7000TH. Results: The disease involved one side of the brain in six patients with six foci, two side of the brain in three patients with seven foci. The shapes of the foci were globular in seven, irregular in six. Five foci enhanced in different forms, eight showed no enhancement after administrating contrast agent intravenously. Conclusion: The CT features of herpes encephalitis simplex are multiple evenly hypointense foci with less space-occupying effects and no contrast enhancement.
