经胸和多平面经食管超声心动图在房间隔缺损介入治疗中的应用

APPLICATION OF TRANSTHORACIC AND MULTIPLANE TRANSESOPHA- GEAL ECHOCARDIOGRAPHY IN THE INTERVENTIONAL THERAPY OF ATRIAL SEPTAL DEFECT

目的 :评价超声引导下经导管或非体外循环经胸小切口以 Amplatzer ASD封堵器治疗房间隔缺损 (ASD)的价值。方法 :3例在经胸超声心动图 (TTE)引导下股静脉穿刺经导管及 1例在多平面经食管超声心动图 (MTEE)引导下非体外循环经右胸小切口置入 Amplatzer ASD封堵器堵闭 ASD。结果 :4例均在超声引导下成功地使用封堵器堵闭 ASD,术中监测ASD的最大伸展径与术前测量基本一致 ,超声可实时指导封堵器准确地置入和堵闭 ASD的操作全过程 ,显示封堵器与周围房间隔咬合完好 ,房水平基本无分流 ,无瓣膜功能和静脉回流障碍 ,即刻评定手术效果。结论 :超声对 Amplatzer ASD封堵器堵闭 ASD的术前病例筛选。

Objective:To evaluate the therapy of atrial septal defect (ASD) by using Amplatzer occlussion device of transcatheter and transthoracic echocardiography without out body circulation Methods:Three patients of ASD were treated with Amplatzer device transcatheter under the guidance of transthoracic echocardiography (TTE) by femoral vein and one patient of ASD was treated with Amplatzer device transthoracic without out body circulation under the guidance of multiplane transesophageal echocardiography(MTEE) Results:Four patients were successfully treated with Amplatzer occlussion device without residual shutting vavular function and venous returned accurately The most extended length measured during operation was consistent with the one prior to operation moniting These showed that the device could absolutely inosculate with the gap of ASD and immediately evaluated the effect of the operation Conclusion:Echocardiography is an important role for selecting ASD case prior to operation,monitoring during the operation and postoperative fellow-up as the patients were treated with Amplatzer occlussion device
