扩张后含真皮下血管网随意型皮瓣血运重建的实验观察

EXPERIMENTAL STUDY ON THE ESTABLISHMENT OF BLOOD SUPPLY OF RANDOM-PATTERN FLAP WITH PRESERVED SUBBCUTANEOUS VASCULR NETWORK AFTER TISSUE EXPANSION

目的 :探讨扩张后含真皮下血管网随意型皮瓣的血运来源 ,蒂的作用 ,纤维囊对皮瓣的影响。方法 :在家猪背两侧埋置扩张器进行皮肤扩张 ,形成长宽比为 5∶ 1的含真皮下血管网随意型皮瓣 ,与未扩张的含真皮下血管网随意型皮瓣进行对比 ,观察其血运重建。结果 :皮瓣血供来自基底、创缘及蒂部 ,蒂部血供的长宽比达 3.5～ 4∶ 1,纤维囊不影响皮瓣的血供。结论 :扩张后含真皮下血管网随意型皮瓣的血供主要来自蒂部 ,长宽比例明显超过未扩张的含真皮下血管网随意型皮瓣 (1～ 1.2∶ 1) ,约 2倍。在皮瓣长宽比例不超过 5∶ 1时 。

Objective:To study the blood supply and the function of pedicle of random pattern skin flap with subdermal vascular network; and the capsular tissue effect on the blood supply of skin flap and it's pathological exchange Methods:Comparision were made of the random pattern subdermal vascular rete skin flaps with a length width ratio of 5∶1 and the skin flaps rased on expanded or non xpanded skin in the pig,and the establishment of blood supply of the flaps were observed Results:The blood supply of skin flaps came from the wound base,the wound margin and the pedicle of the flaps;The flaps with a length width ratio of 3 5~4∶1 survived only with the blood supply of pedicle,and the capsular tissue did not affect the blood supply of flaps Conclusion:The blood supply of random pattern expanded skin;flap with subdermal vascular network mainly came from the pedicle of flaps;the extents of skin flaps viability rased on expanded skin were significantly higher than those of skin flaps rased on normal skin,the capsular tissue did not affect the blood supply of flaps with a length width ratio of no more than 5∶1
