高血压病的微出血

摘要 本文观察了60例高血压病患者的外周微循环,发现高血压病患者大多数伴有微出血,其中球结膜出血达91%,以渗出性出血为主;出血区微血管减少、模糊,微血管内有重度红细胞聚集,甚至有血流停止。微出血随病情加重而加剧。经安定、复方降压片和利尿药治疗,血压正常后,这种微出血并没有终止,仍在继续。这说明上述降压治疗是不足的,还应给予活血化瘀和活血止血治疗。 Microcirculation in the bulbar conjunctiva and nailfold of 60 cases of hypertension and 25 controls were examined. The results showed that microhemorrhage is common disturbance in patients with hypertension. 55 hypertensives (91%) had bleeding in the bulbar conjunctiva and only 1 in the controls (p<0.001). There were 53 cases of oozing hemorrhage (96%) and 2 cases of ruptured vessels (4%). In the hemorrhagic area, patches of hemorrhage without vessels, or with few vessels could be seen. Aggregation of RBC was obvious and blood flow was slow, or even stopped completely. Microhemorrhage correlated with the substages of disease.Following treatment by diazepamum, tabellae reserpinum compositae and hydrochlorothiazidum, blood pressure was lowered to normal, but microhemorrhage in bulbar conjunctiva was still progressing. It suggests that patients with hypertension need to be treated by measures of promoting microcirculation and hemostasis.
