活血化瘀治疗视网膜静脉阻塞的研究

A Study on the Treatment of Retinal Vein Obstruction by Means of Promoting Blood Circulation to Remove Blood Stasis

本文以Q-开关红宝石激光多脉冲辐照法建立家兔眼内出血模型。在中医“留者攻之”理论指导下,拟定活血化瘀复方眼底Ⅲ号口服液对模型眼进行治疗,以尿激酶为对照用药进行实验和临床研究。通过病理学,视觉电生学、血液流变学、体外血栓形成等方面的观察,发现眼底Ⅲ号对家兔眼内出血模型有明显治疗作用,ERGa、b波振幅明显恢复(p<0.05);体外血栓长度明显缩短,重量明显减轻(p<0.01);高低切变率下全血粘度明显降低(p<0.05);病变处巨噬细胞吞噬功能增强(p<0.01)。临床治疗视网膜静脉阻塞95例,眼底Ⅲ号组总有效为41/49眼(83.7%),尿激酶组总有效为27/46眼(58.7%),两组相比有显著性差异(p<0.05)。

In this paper, the rabbit hemalopia model is established by means of Q-switch multi-pulse radiation of ruby laser. Under the guidance of TCM theory that 'if there is stasis, then to remove it', the experemental and clinical study is conducted by taking the self-made oral liquid of Eyeground Ⅲ (E-Ⅲ), which has the effect of promoting blood circulation to remove blood stasis, to treat the schematic eye, and taking urokinase as the control drug. Through the observation in the aspects such as pathology, visual electrophysiology, blood rheology, external thrombosis, etc., it is found that the E-Ⅲ has obvious curative effect on rabbit hemalopia model. The results show that the vibration amplitude of A, B waves in ERG is recovered notably (p<0.05), the length of external thrombus is shorten obviously and its weight decreases notably (p<0.01 ); the total blood viscosity under both high and low velocity gradient decreases significantly (p<0.05); the macrophage phagocytic function in pathological parts increases (p<0.01). 95 cases of retinal vein obstruction are treated clinically. The total curative effect rate of E-Ⅲ group is 83.7% (41/49 eyes), while that of urokinase group is 58.7% (27/46 eyes). There is notable difference between these two groups (p<0.05)
