IDDM患儿的早期肾功能改变

EARLY RENAL FUNCTION IN CHILDREN WITH IDDM

作者测定了26例胰岛素依赖型糖尿病(IDDM)儿童、10例新诊断IDDM儿童及29例正常对照的尿白蛋白排出率(AER)和肾小球滤过率(CFR)水平,发现IDDM患儿AER及GFR明显增高,这种改变在新诊断IDDM患儿中即可观察到。AER与患儿病程有正相关,这种相关主要存在于病程大于5年的患儿。血压升高患儿的AER明显高于正常对照,而血压正常患儿AEK与正常对照无明显区别。在代谢控制不良(HbA_1≥11.5％)患儿中,AER排出增多。未发现IDDM患儿GFR与病程、血压及代谢控制水平的相关。

Urinary albumin excretion rate (AER) and glomerular filtration rate (GFR) were measured in 26 children with IDDM, 10 newly diagnosed IDDM children and 29 non-diabetic controls. The results revealed that AER and GFR in IDDM children were significantly higher than in the controls, which could occur at the onset of clinical diabetes. AER was positively correlated with duration of the disease in diabetics, mainly in those whose illness duration was more than five years. AER level increased in the diabetics with risen blood pressure than in the controls, but no difference was observed between the diabetics with normal blood pressure and the controls. The patients with poor glycemic control (HbA1>11.5%) had higher AER than the controls did, whereas the patients with good glycemic contorl had normal AER. No correlation was found between GFR and duration of disease, blood pressure and indices of metabolic control in the IDDM children.
