摘要
1991年12月以来我们采用真皮下血管网薄皮瓣修复烧伤和外伤后手部疤痕挛缩畸形10例,其中1例合并环指基底节指骨骨髓炎。皮瓣面积8～18cm×4～7cm。断蒂时间最短为术后第7天,最长为12天,平均为8.7天。对真皮下血管网薄皮瓣与真皮下血管网皮片和传统皮瓣进行比较,说明其优缺点,并对轴型和随意薄皮瓣的临床解剖特点及薄皮瓣早期断蒂的成活机理进行了讨论。
Since Dec. 1991, we have used thin skin flaps to reconstruct hand with scar contractures of hand as a result of burn or trauma in 10 patients, including one with osteomyelitis of the proximal phalanx. The size of the skin flaps ranged from 8～18×4×7cm. The shortest time of division of the pedicle was 7 days and the longest was 12 days, with a mean of 8. 7 days. The thin skin flap with preserved subdermal vascular rete was compared with free skin graft, also with preserved subdermal vascular rete, and cnnventional skin flap, and its advantages and shortcomings were discussed. The mechanism of survival after an early division of the pedicle was also discussed.
