Relationship Between Biologic Behavior of Carcinoma of Large Bowel and CEA,EMA Expressions

CEA在大肠癌的表达方式与其生物学行为有关。分化好的癌表达多呈顶缘型或胞浆型,分化差或未分化癌多呈间质型。顶缘型表达出现血管、淋巴管侵犯的较少,而间质型出现侵犯的较多。CEA 进入间质可能是由于癌细胞丧失了正常情况下 CEA 主要在细胞顶缘表达的特性,逐渐分布于细胞的侧缘和基底缘,最后占据整个胞浆,以致通过癌细胞的松散连接漏入间质。EMA 的阳性率与大肠癌分化程度有关,可以作为大肠癌的分化指标。

Immunohistochemical studies revealed that CEA was a product of normal as well as cancerous colonic cells,and that the difference in CEA content between the cancer-ous and normal colorectal mucosa was quantitative but not qualitatlve.The mode of ex-pression of CEA was related to the biological behavior of carcinoma of the large bo-wel.Using immunohistochemical techniques(ABC and PAP methods)the distributionof CEA and epithelial membrane antigen(EMA)in 50 cases of carcinoma of the largebowel was investigated.The results sugessted that the positive rate of CEA was not related to the degreeof differentiation,but demonstrated a good correlation between the degree of differen-tiation and the mode of CEA expression.In well differentiated cancer CEA was pre-dominantly restricted to the apical surface,whereas in moderately and poorly differen-tiated cancers CEA was distributed equally over the entire cell surface and in the su-rrounding stroma.There was a correlation between the mode of CEA expression andpresence of the blood vessel and lymphatic invasion.It was also shown that EMA was a good indicator for the degree of differentiationof carcinoma of the large bowel.
