小肠平滑肌肿瘤

SMOOTH MUSCLE TUMORS OF THE SMALL INTESTINE

本文报告60例小肠平滑肌肿瘤,其中平滑肌瘤19例,平滑肌肉瘤41例。腔外型49例(81.7％),腔内外型6例(10％),腔内型3例(5％)和壁间型2例(3.3％)。主要临床表现为腹块(40例)、出血(31例)和腹痛(29例)。病人可分成三种临床类型:出血型(43.3％)、腹块型(35％)和急腹症型(21.7％)。小肠平滑肌肿瘤的早期诊断困难,纤维内镜、小肠灌钡和低张造影、吞线检查和腹部CT检查可有所帮助,选择性血管造影对早期诊断具有重要的价值。

Sixty cases of smooth muscle tumors of the small intestine are reported. There were 19 cases of leiomyoma and 41 leiomyosarcoma. The tumors were extraluminal in 49(81.7%), extra- and intra-luminal in 6 (10%), intraluminal in3(5%)and intramaral in 2 (3.3%). The chief clinical presentations included abdominal mass (40 cases), bleeding(31 cases) and abdominal pain (29 cases). Therefore the patients were classified into: (1) gastrointestinal hemorrhage (43.3%); (2) abdominal mass (45%); (3) acute abdomen (21.7%). Early diagnosis of smooth muscle tumors of the intestine is difficult. Fiberoptic endoscopy, barium meal and hypotonic small bowel examination, string swallowing test and abdominal computed tomography might be of value. Selective angiography has an important role in the early diagnosis of small intestine tumors.
