水平前侧喉切除治疗双声带癌的经验

EXPERIENCE IN THE TREATMENT OF CARCINOMA OF BILATERAL VOCAL CORDS WITH HORIZONTO-FRONTAL LARYNGECTOMY

本文报道水平前侧喉切除治疗３０例双侧声带癌的经验。介绍了手术切除及扩大切除的范围；一侧用颈部带蒂皮瓣，一侧用肌膜瓣或梨状窝粘膜等进行整复，重建喉室、声带、前连合及扩大切除杓状软骨者，填补杓状软骨缺损的方法。并对减少组织损伤，保留神经血管束，重建喉的解剖形态，提高疗效的经验进行讨论。

patients with cancer of bilateral vocal cords were treated with horizonto-frontal laryngectomy from 1981 to l992. For the patients with T2 or T3 lesions extented excision should be performed, if necessary. The defect of laryngeal cavity after tumor removal was repaired and the reconstructive vocal·cords, ventricular band and an-terior commissure were made. Good quality of voice was obtained and the function of larynx was restored quickly after operation in the majority of cases. One patient, who was a teacher at a middle school, was able to teach again. workers or peasants can do heavy physical labour after treatment. of 30 patients one died from other cause but free of tumor four years after partial laryngectomy. The remaining 29 patients are still alive with no signs of recurrence for mean 4.6 years (range 2 to 10 years) follow up.
