不同类型高血压性左室肥厚的心功能改变

CHANGES OF CARDIAC FUNCTION IN PATIENTS WITH ESSEN-TIAL HYPERTENSION WITH DIFFERENT PATTERNS OF LEFT VENTRICULAR HYPERTROPHY

本文采用超声心动图法对９７例高血压合并左室肥厚（ＬＶＨ）的心脏结构和功能改变进行研究，结果显示：（１）高血压各组舒张功能都有减退，以向心性肥厚组（ＣＨ）减退最为显著，（２）心肌收缩性（ＳＢＰ／ＬＶＤｓ／ＰＷＴ）高血压无肥厚组升高，ＣＨ组和不对称性室间隔肥厚组（ＡＳＨ）正常，但左室扩张组（ＤＨ）明显减退并伴有收缩末期室壁应力的异常升高.提示高血压左室肥厚时不同类型左室形态结构的改变对心室收缩和舒张功能均产生重要影响.

The changes of cardiac function were observed through eckocardiagraphy in 97 pa-tients with essential hypertension ( EH ) with different patterns of left ventricular hypertrophy(LVH). The results showed : (l)The diastolic performance was found to be decreased in all hyper-tensive patients,especially in concentric hypetrophy(CH)group.(2)The myocardium contactility(SBP/LVDs/PWT)was found to be supernormal in EH without LVH group,normal in CH andASH(asymmetric septal hypertrophy) groups,but decreased in diastolic left ventricular hypertrophy group(DH)with significant increase in ESS.It suggested that the changes of cardiac structure might have important effect on cardiac systolic and diastolic performance in EH。
