人肝细胞性和胆管细胞性肝癌的细胞角蛋白表达

The expression of cytokeratins in human hepatocellular and cholan- giocellular carcinomas

为了明确用胆管型细胞角蛋白（ＣＫ）抗体在肝细胞性（ＨＣＣ）和胆管细胞性肝癌（ＣＣ）的鉴别诊断中的应用价值，我们用能在肝内特异性识别ＣＫ１９和ＣＫ１８的抗体及层粘连蛋白（ＬＮ）抗体进行了免疫组织化学研究．结果表明，１０例ＣＣ全为ＣＫ１９阳性，３７例ＨＣＣ中１４例（３８％）有ＣＫ１９阳性癌细胞．因此，我们提出ＣＫ１９表达与否不能作为鉴别ＨＣＣ和ＣＣ的标准．ＨＣＣ中ＣＫ１９表达与其分化程度无关，但与其组织学类型有关．呈腺样分化的癌巢全呈ＣＫ１９阳性，并且这种癌巢都有不连续的ＬＮ阳性基底膜样结构，显示ＨＣＣ中的腺样分化、ＣＫ１９表达也与ＬＮ有关，类似良性肝脏病变及胚胎肝。

In order to determine the applied value of the antibodies to cytokeratins(CKs )of bile duct type inthe differential diagnosis between hepatocellular carcinoma(HCC)and cholangiocellular carcinoma(CC ),we have made an immunohistochemical investigation,using the antibodies specifically recognizing CK 19 and CK18,separately,in liver and laminin(LN ) antibody.All the CCs examined( 10 cases)were found CK19-positive; interestingly,CK19- positive cancer cells were also observed in 38%of HCCs( 14 /37).Therefore,CK19 was not a reliable marker in differenting HCC from CC, in our consideration.The CK19 expression in HCC was showed to be irrelevant to their differentiation degree,but related to the histologic subtypes whichindicated the directions of their differentiation.CK19 expression was observed in all the HCC cell nests withglandular differentiation,and an uncontinuous LN- positive basement membrane- like structure was immunolo- calized around these cells,which indicated that the glandular differentiation and CK19 expression in HCCwere also related to the LN deposition,as in fetal liver and some chronic liver disorders.
