胰腺外瘘诊治的进一步体会

Advanced experiences on treatment of pancreatic external fistula

就７例因胰岛细胞瘤手术及胰腺损伤后形成胰瘘的资料提出防治意见．并介绍了对２例长期不愈的胰瘘患者经直接Ｒｏａｘ－ｅｎＹ式瘘管空肠吻合手术获得满意疗效的经验，认为病灶残腔不能紧密缝合时，局部使用鱼肝油酸钠及凝血酶，更好地使用引流、必要地应用尿激酶及术后使用５—Ｆｕ对预防胰瘘的发生起着一定的作用．

The experiences of diagnosis and treatment of 24 patients with pancreatic external fistula have been reported in 1986. Although there are some improvements recently, the therapeutic effect is not satisfactory yet. Because of harmful results of pancreatic fistula, authors now reporta way to prevent from occurrence of pancreatic fistula during the operation on islet cell tumor, and satisfactory effect of treatment was obtained by direct Roux-en Y in 2 patients with chronic unclosed pancreatic fistula pancreaticojejunostomy.As to the prophylaxis of this complication, authors pointed out the importances of suture untightness for residual cavity, local medication of sodium morrhuate and thrombin, better use of drainage and application of urokinase if necessary. Postoperative medication of 5-Fluorouracilum has certain effects for prevention from occurrence of pancreatic fistula.
