摘要
TNF主要是由单核一巨噬细胞产生的一种多功能性细胞因子,对肿瘤细胞具有直接杀伤作用。1993年6月至1994年2月,作者应用TNF一S治疗大肠癌13例,结合资料,本文对TNF-S在大肠癌临床应用中的效果进行了探讨。
Tumor necorsis factor(TNF)is one of the cytokines produced mainly by themononuclear cells(macrophages).Recently,using gene engineering,highly purified rTNF-α hasbecome available in large quantity.As rTNF-α showed direct or indirect antitumor activities invivo,clinical trials using r一TNF一α to regress suppress tumors had been conducted in manycountries.However,he results of these clinical trials were disappointing in view of the reportedanti-tumor effects.One ofthe major problems affecting clinical responces may be that rTNF-αcannot be administered in high dosage(over 1 x 106u/body)because of the side effects,such asthrombocytopenia,hypotension,ete.In order to obtain novel TNF which produces lesser side effects bat retains its anti-tumoreffects,various muteins have been constructed by gene engineering,and their biological activi-ties analysed.Amony these muteins,rTNF-SAM2 in which the bnsicity of the N一terminal re-gion is higher than that of rTNF-α,is found to be most promising in view of its antitumor ef-fects and its lesser side effects.From June 1993 to Feb.1994,we had conducted clinical trials using rTNF一SAM2 mainly inpatients with colorectal tumor,including patients with metastatic colorectal tumor in the lungsand lor liver In all,13 patients had been treated with systematic administration ofrTNF一SAM2.Basing on the results obtained, the authors discussed the clinical usefulness ofrTNF-SAM2 in view of both of its anti tumor effects and side effects.
