胃十二指肠溃疡患者壁细胞体视学分析及其意义

A MORPHOLOGICAL STUDY Of PATHOGENESIS OF PEPTIC ULCER AND DUODENAL ULCER

应用电子计算机图像分析处理系统对胃溃疡（ＧＵ）和十二指肠溃疡（ＤＵ）患者壁细胞进行立体定量学分析，并在光镜下对胃粘膜厚度进行测量。发现ＧＵ、ＤＵ和基本正常胃粘膜（ＮＳ）三者间壁细胞平均剖面直径（Ｄ）、平均剖面积（Ｓａ）、平均体积（Ｖｖ）、体积密度（Ｖｔ）和数量密度（Ｎ）无明显差别（Ｐ＞０．２）：而ＤＵ组胃粘膜厚度明显大于ＧＵ组和ＮＳ组（Ｐ＜０．００１），ＧＵ组和ＮＳ．组无明显差别（Ｐ＞０．０５）。提示：ＤＵ和ＧＵ发生机制不同，ＤＵ的发生可能与高胃酸分沁量有关，而ＧＵ则可能由胃粘膜屏障机能减退所致。

Model VAX11/750 computer connected with Model 75 image analysis system was used to carry out stereoquantitative analysis of parietal cells of patients with peptic ulcers, and the thickness of the gastric mucosa was measured with L M. The results showed that D, Sa, Vv, Vvt and Nv of Parietal cells were not significantly different between DU, GU and NS (P<0.2 ) ;but the gastric mucosa of patients with DU was markedly thicker than that of patients with GU or with NS (P<0.001 ). It is suggested that DU is probably caused by the hypersecretion of gastric acid, and GU by the deteriorated function of the gastric mucosal barrier.
