旁正中穿质及其动脉的显微外科解剖

MICRO SURGICAL ANATOMY OF THE PARAMEDIAN PERFORATED SUBSTANCE AND ITS ARTERIES

用手术显微镜观测了48侧旁正中穿质及其动脉.旁正中穿质是界于乳头体、灰结节、视束与大脑脚之间的三角形区域,穿入此区的动脉为旁正中穿动脉和乳头体前动脉.乳头体前动脉的出现率97.92%,主要起自后交通动脉(94.23%),其余起自颈内动脉和大脑后动脉的交通后段.乳头体前动脉双支的出现率为10.42%.有80.77%的乳头体前动脉发支至视束、大脑脚和旁正中穿质.详细观察了穿旁正中穿质的动脉外径、长度及行程.并结合临床有关问题进行了讨论.

The paramedian perforated substance and its arteries were observed and measured in 24 adult brains by operation microscopy. The paramedian perforated substance was shaped almost like an equilateral triangle bounded by the mamillary body and tuber cinereum, the optic tract, and the cerebral peduncle. The arteries entering the paramedian perforated substance were divided into the paramedian perforating artery and the premamillary artery. The premamillary artery occured in 97. 92% of cases, 94.23% of the premamillary arteries arose from the posterior communicating artery and the rest from the internal carotid artery and the posterior cerebral artery 80.77% of the premamillary arteries branched to the optic tracts, cerebral peduncles, and paramedian perforated spaces. The outer diameter, length, course of the arteries entering the paramedian perforating substance were investigated in detail.
