摘要
作者分析经心内电生理检查证实的房室折返性心动过速(AVRT)60例及房室结折返性心动过速(AVNRT)14例的心电图逆行P波(P’),发现P’在Ⅱ、Ⅲ、aVF导联的出现率高于其它导联(p<0.05),P'及R-P’间期≥70ms诊断AVRT的敏感性为90%,特异性分别为93.1%和98.1%;I导联负向P’诊断左侧房室旁道(AP)的敏感性为87.2%,特异性为100%。根据P’与R-P’间期及I导联负向P’区分AVRT与AVNRT及时AP初步定位准确性高,简单方便迅速,具有较大的价值。
Authors evaluated electrocardiography of 60 cases with atrioventricular reciprocat-ing tachycardia (AVRT )and 14 cases with atrioventricular nodal reciprocating tachycardia (AVNRT)diagnosed by intracardiac electrophysiologic study. Results shown that there were the more retrogradeP waves ( p’)in lead Ⅱ.Ⅲ and aVF than others(P<0.05).The sensitivity of p’ and R-P’intervaldiagnosing AVRT was 90%,specificity 93.1%and 98.1%respectively. Sensitivity of negative p’ inlead I diagnosing left atrioventricular accessory pathway (AP) was 87.2%,specificity 100%. P’,R-P’interval and negative p’ in lead I are a noninvasive, accurate, single and rapid way identifyingAVRT from the others and localizing the left AP.
