狼疮性肾炎组织形态学半定量分析及其临床意义

Histomorphologic Semiquantitative Indexes of Patients with Lupus Nephritis and its Clinical Significance

对26例狼疮性肾炎(LN)肾穿刺活检标本组织形态学进行半定量分析,结果表明:LN组织学类型、肾功能状况与活动性指数(AI)、慢性指数(CI)有明显关联。进一步将半定量指数分为三级,发现AI 0～6分的病人很少发生肾功能衰竭(1/13),>10分的病人均发生肾功能衰竭(5/5),7～10分者1/4病人发生肾功能衰竭(2/8)。CI分级不如AI分级敏感。认为LN组织形态学半定量分析,对判断预后、指导治疗可能具有重要意义。

The histomorphologic features of the renal biopsy specimens of 26 patients with lupus nephritis (LN) were studied with a semiquantita-tive technique. The results showed that the his-tologic types and the renal function of patients with LN correlated with the activity index(AI) and the chronicity index(CI). The semiquantita-tive indexes were further divided into three grades. It was found that the patients with AI between 0 to 6 rarely occured renal failure (1/ 13), however, all 5 patients with AI more than 10 and 1/4 patients (2/8)with AI 7 to 10 had suffered from renal failure. The histomorphologic semiquantitative analysis mignt have an important role on the prognostic evaluation and instructive treatment of the patients with LN.
