寰枢椎后路固定融合术治疗寰枢椎脱位

TREATMENT OF ATLANTOAXIAL DISLOCATION BY ATLANTOAXIAL ARTHRODESIS

本组报告１５例寰枢椎前脱位，以颈椎外伤及先天性发育畸形等原因所致，其中外伤性齿状突骨折伴寰枢椎脱位１２例，齿状突发育畸形伴寰枢椎脱位３例，术前均行颅骨牵引复位后行寰椎后弓钢丝贯穿、寰枢椎固定加植骨术，１３例获得解剖复位，随访时间平均１０个月以上，Ｘ线片检查１２例植骨已融合。

Fifteen patients of atlantoaxial dislocation caused by injury and congenital defor- mity were reported in this article,in which traumatic dens fracture with atlantoaxial dislocation was showed in 12 cases and congenital dens deformity in 3 cases.The at- lantoaxial facet jiont was fixed with wire passing beneath the spinous process and around the arch of the atlas. A bone graft was placed between the arch of the atlas and the spinous process of the axis.The average following time was 10 months.The results showed a satisfactory reduction of the atlantoaxial facet joint with a high rate of the bone graft fusion.
