CT对急性坏死性胰腺炎的分类与预后评价

CT CLASSIFICATION OF ACUTE NECROTIC PANCREATITIS IN RELATION TO ITS PROGNOSIS

坏死性胰腺炎为急性胰腺炎的严重形式，其特征为胰腺实质坏死和并发症多，死亡率高，缺乏特异性诊断方法和估计预后的指标。本文根据１２２例ＣＴ表现，提出一个分类方法并观察其与预后的关系。其中，Ⅰ类３１例，无并发症，预后佳；Ⅳ类并发症多，预后差，６例均死亡；Ⅱ、Ⅲ类８５例，死亡２３例，占２７．０６％。本文结果显示ＣＴ可作为评估预后的方法。

ecrotic pancreatitis is a serious type of acute pancreatitis. It is characterized bynecrosis of parenchyma of the pancreas and invading of the surrounding tissue withvarious complications and high mortality.There is no special diagnotic proceduresand prognostic prodictors for this disease.Here a CT classification of the disease isproposed according to our analysis of 122 cases.31 cases were classified as group Ⅰ,without any complication and with good outcome.6 cases were classified as group Ⅳ. All of them died due to sevcre complications.85 cases belonged to group Ⅱ and groupⅢ,in which 23 cases(27.06%)died.The result of this study indicated that CT scancould be used to predict the outcome of acute neerotic pancreatitis.
