瓦氏动作对正常人心脏结构和血液动力学的影响——二维及脉冲多普勒超声心动图研究

摘要 应用二维及脉冲多普勒超声心动图观测10例正常人瓦氏动作过程中心脏结构和血液动力学的改变。结果表明:瓦氏动作Ⅱ期心腔缩小、每搏量减低,心率加快,而瓦氏动作Ⅳ期出现一个明显高于动作前的每搏量及射血分数。此动作对二尖瓣血流的影响表现为:E峰流速、E/A比值及峰值充盈率减低,E峰加速时间及减速时间延长。 Two-dimensional and pulsed Doppler echocardiography was used to evaluate the changes of cardiac size and hemodynamics during Valsalva maneuver in 10 normal subjects. The results showed a significant decrease in cardiac chamber size, SV and an increase in HR during the strain phase of the maneuer (phase II). They returned to the baseline soon after release of the Valsalva maneuver (phase IV) with a transient surge of increased SV and EF above that of the resting state. The effect of the Valsalva maneuver on the transmitral flow velocity profile was that E velocity, E/A ratio and peak filling rate were decreased but acceleration half time and deceleration half time were increased.
