The Activities of Serum Enzymes in Ischemic Hepatitis

摘要 研究九种血清酶在缺血性肝炎时的活性变化,并与无肝损害的心肌梗塞病人进行对照。发现谷氨酸脱氢酶(GDH)在缺血性肝炎时最敏感,上升幅度最大,特异性亦强。转氨酶虽然非常敏感,但缺乏特异性。鸟瞟呤脱氨酶(GD)和谷胱甘肽S-转移酶(GST)在缺血性肝炎时敏感性、上升幅度均优于乳酸脱氢酶(LDH),且特异性亦为转氨酶及LDH所不能比拟。γ-谷氨酰转肽酶(GGT)对缺血性肝炎无确切临床价值。提示GDH应作为缺血性肝炎的特异性诊断指标。 The changes of nine serum enzyme activities in patients with ischemic hepatitis were studied and those in acute myocardial infarction without liver damage were compared. The serum glutamate dehydrogenase (GDH) was the most sensitive indi-cator,GDH level rose up most significantly in hepatitis patients and it had good specificity for liver disease. Although serum aminotransferases were very sensitive, they had a poor specificity in liver disease. Guanase (GD) and glutathione s-trans-ferase (GST) had better sensitivity and more increase than lactate dehydrogenase (LDH). Their specificity in liver damage (ischemic hepatitis) was much better than that of aminotransferases and LDH. Gamma glutamyl transferase (GGT), alkaline. Phosphata'se (ALP) and creatine kinase (CK) were not helpful for diagnosing ischemic hepatitis. It is suggested that GDH may be the most sensitive and specific serum marker in ischemic hepatitis.
