胃癌和胃癌前病变血浆和胃液物质P、血管活性肠肽、亮脑啡肽研究

Substance P, Vasoactive Intestinal Peptide and Leu-enkephalin in Plasma and Gastric Juice in Patients with Precancerous Lesinos or Gastric Cancer

研究慢性萎缩性胃炎伴癌前病变(n=28)和晚期胃癌(n=22)血浆和胃液某些脑肠肽含量。以33例正常人作对照。血标本和胃液标本分别于空腹和胃镜检查时收集。血浆和胃液物质P(sP)、血管活性肠肽(VIP)和亮脑啡肽(LEK)均用放射免疫法测定。结果胃癌患者血浆、胃液SP和LEK、胃液VIP含量明显升高(P<0.05),血浆SP和LEK分别与胃液SP和LEK呈正相关(r=0.432和0.516,P<0.05)。胃癌术后血浆SP和胃液LEK明显低于术前(P<0.05)。胃癌前病变患者,血浆和胃液LEK,胃液VIP升高(P<0.05),且血浆胃液LEK呈正相关(r=0.398,P<0.05)。本研究提示联合测定血浆胃液SP、VIP和LEK对研究胃病时胃肠激素有一定临床意义。

To investigate the changes of brain-gut peptide concentrations in the plasma and gastric juice and their relation to gastric diseases, this study conducted on chronic atrophic gastritis with precancerous lesions (n = 28) and gastric cancer in advanced stage (n= 22),and normal subjects .(n = 33) as controls. Samples of blood and gastric juice were collected at fasting and under gastroscopy respectively. Plasma and gastric juice substance P (SP), vasoactive intestinal peptide (VIP) and leu-enkephalin (LEK) were measured by radioimmunoassay kit. In cases with gastric cancer, SP and LEK levels of plasma and gastric juice, and VIP of gastric juice were higher than in the controls (P<0.05), SP and LEK levels of plasma correlated with those of gastric juice (r= 0.432 and 0.516, P<0.05); in the postsurgical gastric cancer cases plasma levels of SP and gastric juice LEK were lower than those in presurgi-cal period (P<0.05). In cases with precancerous lesions, plasma and gastric juice LEK levels and gastric juice VIP level were increased(P<0.05, respectively), plasma LEK level correlated with the gastric juice one (r = 0.398, P<0.05). These data show that the measurement of concentrations of SP, VIP and LEK in plasma and gastric juice is of clinical significance for study on gastrointestinal hormones in stomach diseases.
