产超广谱β-内酰胺酶肠杆菌科细菌感染的预防和治疗 被引量:2

摘要 1987年Bauernfeind首次报道大肠埃希菌中耐药质粒编码能水解头孢菌素类抗生素的β-内酰胺酶,掀开了对超广谱β-内酰胺酶(ESBLs)研究与监测的序幕.在过去十几年,全球已多次爆发产ESBLs细菌感染,目前,有关ESBLs的确切流行情况尽管尚不清楚,但肯定是在增加,在全球很多地区,多达10% ~ 40% 的大肠杆菌和肺炎克雷伯杆菌菌株产ESBLs.在我国浙江地区,大肠杆菌和肺炎克雷伯杆菌菌株产ESBLs的发生率分别为34%和38.3%[1].在东欧和拉丁美洲的一些国家,肺炎克雷伯杆菌产ESBLs的发生率高达50%[2].ESBLs能够水解青霉素、广谱头孢菌素和单环β-内酰胺抗生素,导致这些抗生素临床使用无效.产ESBLs肠杆菌科细菌对多种抗生素产生耐药性,已成为控制感染所面临的挑战.
