幽门螺杆菌感染对血浆一氧化氮及食管动力的影响

摘要 目的 探讨幽门螺杆菌 (Hp)感染对血浆NO及食管动力学的影响和可能的机制。 方法 连续观察 92例胃炎者Hp感染、血浆NO测定和食管运动功能。 结果  5 4例Hp感染者和 3 2例非Hp感染者血浆NO浓度分别为97.0 0 (113 .3 5 )和 49.82 (2 6.5 0 ) ,差异有显著性 (P =0 .0 2 3 ) ;UESP、LESP和食管体正常蠕动分别为 2 9.47(12 .2 6)、17.2 4(7.5 9)、5 .0 4(4 .0 7)和 3 6.2 3 (14 .85 )、16.95 (7.78)、7.0 0 (3 .0 0 ) ,差异无显著性 (P =0 .15 0 4、0 .912 4、0 .12 5 4)。 19例Hp感染根除前后血浆NO浓度、LESP分别为 13 6.18(182 .78)、14 .10 (8.16)和 43 .10 (17.95 )、18.71(8.2 2 ) ,有较显著的改善 (P =0 .0 3 7、0 .0 0 1) ;UESP及食管体正常蠕动为 3 5 .87(13 .5 6)、5 .3 2 (3 .79)和 3 8.63 (16.95 )、6.2 1(3 .43 ) ,则无明显变化 (P =0 .412、0 .112 )。结论 Hp感染可导致血浆中NO含量明显高 ,但对食管动力的影响可能不仅仅是导致血浆NO浓度增高这一单因素作用。而根除Hp后LESP可有明显的升高。 Objective To explore the effect and possible mechanism of Helicobacter pylori(Hp) infection on serum Nitric oxide(NO) and motor function of esophagus.Methods Hp infection,serum NO and motor function of esophagus were determined in 92 patients with chronic gastritis.Results The serum NO of 54 cases with Hp infection was significantly higher(97.00±113.35) than 32 without(49.82±26.50).There was no significant different in UESP,LESP and esophageal body peristalsis.There was a significant improvement in NO(136.18±182.78 vs. 43.10±17.95, P=0.037) and LESP (14.10±8.16 mmHg vs. 18.71±8.22 mmHg, P=0.001), but not in UESP(35.87±13.56 mmHg vs 38.63±16.93 mmHg, P=0.412) and esophageal body peristalsis(5.32±3.79 vs 6.21±3.43, P=0.112) before and after Hp eradication in 19 patients.Conclusion Hp infection may be result in increasing the content of serum NO. But the effect on esophagus motor function was not increased content of NO. There was a significant improvement in LESP after Hp eradiation.
